Texas Client Rights: Access to Records & Complaints
This page provides information about (1) requesting health care records, (2) contacting the applicable licensing authority, and (3) filing a consumer complaint, in accordance with Texas Health & Safety Code §181.105.
1) Requesting Your Health Care Records
To request your records from [PRACTICE / CLINIC NAME], please follow these steps:
- Submit your request
- Email: [RECORDS REQUEST EMAIL]
- Phone: [PHONE NUMBER]
- Mail: [MAILING ADDRESS]
- Include the following information
- Your full name and date of birth
- Your preferred phone number and email address
- The type of records you are requesting (for example: intake paperwork, treatment summary, billing receipts)
- The date range of services (approximate dates are okay)
- Where you would like the records sent (you, another provider, or a third party)
- Any deadlines we should be aware of
- Authorization / verification
For your privacy, we may request identity verification and/or a signed Release of Information before releasing records to you or to a third party.

3) Filing a Consumer Complaint (Texas Attorney General)
You may file a consumer complaint with the Texas Office of the Attorney General (Consumer Protection Division):
