Texas Client Rights: Access to Records & Complaints

This page provides information about (1) requesting health care records, (2) contacting the applicable licensing authority, and (3) filing a consumer complaint, in accordance with Texas Health & Safety Code §181.105.

1) Requesting Your Health Care Records

To request your records from [PRACTICE / CLINIC NAME], please follow these steps:
  1. Submit your request
    • Email: [RECORDS REQUEST EMAIL]

    • ‍Mail: [MAILING ADDRESS]
  2. Include the following information
    • Your full name and date of birth‍
    • ‍Your preferred phone number and email address
    • The type of records you are requesting (for example: intake paperwork, treatment summary, billing receipts)
    • The date range of services (approximate dates are okay)
    • Where you would like the records sent (you, another provider, or a third party)
    • Any deadlines we should be aware of
  3. Authorization / verification

For your privacy, we may request identity verification and/or a signed Release of Information before releasing records to you or to a third party.
